Machine Repair Request – Maxx, Maia, Semi 3 & 5, Semi 5, Duo (Vesta Duo), B2

If your machine needs to be repaired please fill out this form you will receive an email with an address to ship machine and a repair # as well as shipping instructions.


Machine Repair Steps:

STEP 1: If you have a Maxx, Maia, Maia Duo, Semi 5, Semi 5 Duo (formerly Vesta Duo), Semi 3, or B2 machine you need to complete the form below.

STEP 2: Once the below form is completed you will need to complete The Credit Card Authorization form on the next page.

STEP 3: When both forms have been submitted, you will receive an email within 1 to 2 business days. This email will include an RMA number, and an address with instructions on where to ship your machine. DO NOT ship your machine until you receive the RMA instructions email. If you do not see this email in 48 hours please check your junk and spam. If you have questions please call 833-MagnaWave

  • Please Choose a machine from the Drop Down Box below. Example: Maxx, Maia, Semi or Vesta Duo. The Pulse Pro, Julian & Sol have another repair form; if you need one of these repaired do not complete this form.
    What is going on with the machine or attachments?
  • Please give as much detail as possible.
  • Your machine Serial Number is located on the lower corner on the inside of the door of all All-Terrain and Traveler machines. It is on the bottom of the office machines. It is usually a 5 digit number. This number is required before your machine can be repaired.
  • If you would like to rent a machine for the time your machine is out, that is an option, but ONLY if we have loaner machines available. LOANERS are $100 a week + shipping if the loaner is not returned Promptly when your repaired machine arrives back to you then you will be Charged additional Fees. To check on loaner availability please call our office 1-502-742-7868 . LOANERS are not guaranteed. Please indicate wether you would like a loaner machine below.
  • When your machine is repaired wher would you like it shipped too.
  • You must complete the Credit Card Authorization Form for your machine to be returned to you. Magna Wave is not responsible for returning any machines by a certain date, If you are in a rush please choose expedited return shipping. Depending on repairs needed, machine service can take 2 to 7 business days + shipping.
  • VERY IMPORTANT - TO COMPLETE REPAIR REQUEST CLICK SUBMIT - Then complete the form on the next page! If this form is not completed you will not recieve your RMA and your repair time may take longer.

    AGAIN If you do not complete the form on the next page YOU WILL NOT GET a machine REPAIR number.
